On March 14, 2022, EFMP and OSS are moving from the Neurology clinic, Sunrise Pavilion, Floor 3, Reception 1 to River Pavilion, Floor 1, Reception 2. Navy EFMP and OSS will remain in Neurology with updated hours of operation.

Overseas Suitability Screening

The purpose of an Overseas Suitability Screening is to identify medical, dental, educational and potential duty-limiting conditions or requirements of both service and family members. Completing a screening will ensure that both the family and service member are qualified for overseas, operational duty, and remote duty assignments. For the best medical care or education, sometimes an overseas tour is not in the service or family member's best interest.

Prior to scheduling an OSS appointment please ensure the following:

Active Duty

  • Periodic Health Assessment and Flight Physical (if applicable) are up to date.
  • Dental classification 1 or 2 (dental classes 3 and 4 are non-deployable).
  • For females, pap smear needs to be current within one year, with normal results.
  • Females 40 years or older require a mammogram.

Dependents

  • For children a well-child visit needs to be current within a year of transferring date.
  • For females a pap smear needs to be current within two years of transferring date.
  • Females 40 years or older require a current mammogram.
  • If dependents are seen by a civilian provider, patient needs to provide a copy of the last annual exam, current copy of immunizations, results of last pap smear, last mammogram results (only if dependent is 40 years or older) and notification stating there are no medical or mental health concerns that require mandatory Exceptional Family Member Program enrollment.
  • If dependents are currently enrolled in the EFMP program, enrollment must be current within the last 3 years.

In addition to the forms listed at right, if dependents are seen by a civilian provider, a medical clearance form, and physical exam form must be completed by the civilian provider.

Service-Specific Overseas Suitability Screening Requirements

Air Force

  • Must have recent physical exam (for all family members) within the past six months.
  • Must have an AF Form 1466 and DD Form 2792 completed by service member/spouse and PCM for each family member identified with special medical/education needs prior to appointment.
  • Must have completed AF Form 1466D for each family member older than two years with unresolved dental needs or dental exam in the past six months.
  • Must have a DD Form 2792-1 completed by school for all children 3 years and older and/or one completed by Early Intervention services for all children 0 to 35 months receiving Early Intervention services.

For EFMP enrollees only

  • EFMP enrollment must be updated with each new permanent change of station. See USAF EFMP link in the information box.

Army

  • Must have recent physical exam within the past 12 months.
  • Must have completed DA Form 5888 with Section 8 completed by the service members’ personnel office.
  • Must have completed DA Form 7246 (EFMP Screening Questionnaire) by service member or spouse.
  • If receiving care from a civilian provider an EFMP physical examination form and Medical clearance statement form are required. (Please contact EFMP Coordinator for copies of these forms).

For EFMP enrollees only

  • EFMP enrollment must be current within the past three years.
  • If updating EFMP enrollment, DD Form 2792 must be completed by service member/spouse and PCM prior to appointment.
  • For children with special education needs, DD Form 2792-1 must be completed by their school.
