DiLorenzo Pentagon Health Clinic Medical Readiness
This clinic conducts physical exams for service members assigned to the Pentagon. We provide annual Periodic Health Assessment (PHA), Separation History and Physical Exam (SHPE), Special Duty Screenings, and Deployment Health Assessments. NOTE: Our clinic does not complete Navy overseas screenings.
Annual Readiness Requirement
Depending on your branch of service, you will refer to your IMR, MOL, BOL or Airforce portal to determine your readiness status. Please review your immunizations, vison screening, audiology, laboratory, and dental prior to your PHA. Ensure you are “green” in all categories. Your age and what documentation you need signed will determine what categories apply to you in the listing below. Your readiness status directly affects the ability for a technician or provider to sign documents you may need.
Periodic Health Assessment
Annual physical health assessments must be completed to be considered medically ready by a provider. Use the links below to access the online portion of your PHA. This online portion must be completed prior to scheduling with one of our providers to complete your assessment.

These exams may not be completed prior to 180 days before your final out.
- Service members preparing to retire may come to Medical Readiness 180 days prior to retirement to obtain a retirement packet for completion (DD Form 2807-1 and DD Form 2808).
- A packet must be filled out and signed by Dental prior to scheduling this appointment.
- Once packet is signed and completed, return it to the Medical Readiness office. Labs and chest X-ray will be ordered. Then your SHPE will be scheduled.
- If your labs and chest X-ray are not completed prior to your appointment, you will be rescheduled for a new appointment.
NOTE: Elective non-urgent surgery must be completed prior to scheduling SHPE. Persons participating in skillsbridge program will be scheduled accordingly on a case-by-case basis.
Frequently Asked Questions
Radiology
- Hours: Monday–Friday, 7 a.m. to Noon and 1 to 4 p.m.
- Radiology is available on a walk-in basis once an X-ray is ordered and signed off by a provider. After submitting your packet for retirement, a chest X-ray will be ordered by Medical Readiness staff at the time of scheduling your final physical.
Immunizations
- Hours: Monday–Friday, 7 a.m. to 4 p.m.
- Screenings and administration of immunizations are available on a walk-in basis.
Vision Screening
- Hours: Monday–Friday, 7 a.m. to 3:30 p.m.
- Vision readiness screenings are conducted by the Medical Readiness Clinic. If you wear contacts, remove them 24 hours prior to the screening and bring your glasses. Updating your vision does not require a visit to Optometry.
- If you wear corrective lenses and need a new set of glasses, mask inserts, a new prescription, or its been over two years since your last optometry exam, contact Optometry to make an appointment.
Audiology
- Hours: Monday, Tuesday, Thursday, and Friday, 7:30 to 11:30 a.m. and 1 to 2:30 p.m.
- Audiology is available by walk-in basis only. Please sign into medical readiness upon your arrival. Final audiograms must be completed within 6 months of retirement/separation date. If you are on flight status, you can also call Flight Medicine at 703-697-3255.
Laboratory
- Hours: Monday–Friday, 7 a.m. to Noon and 12:30 to 3 p.m., with limited services from 3 to 4 p.m. for STAT blood draw requests only.
- HIV is due every 2 years and can be drawn at a walk-in basis at our laboratory. It does not require an order from your physician.
- Deployment: Must be current within 6 months of your departure.
Dental

Colonoscopy
