DiLorenzo Pentagon Health Clinic Medical Readiness
This clinic conducts physical exams for service members assigned to the Pentagon. We provide annual Periodic Health Assessment (PHA), Separation and Retirement Physicals (SHPE), Special Duty Screenings, and Deployment Health Assessments. During our administrative time, providers review and sign documents required for PCS’ing, Reenlistments, and Reassignments.
Annual Readiness Requirement
Depending on your branch of service, you will refer to your IMR, MOL, BOL or Airforce portal to determine your readiness status. Use the information below as a reference to ensure you are “green” in all categories. Your age and what documentation you need signed will determine what categories apply to you in the listing below. Your readiness status directly affects the ability for a technician or provider to sign documents you may need.
Periodic Health Assessment
Annual physical health assessments must be completed in order to be considered medically ready by a provider. Use the links below to access the online portion of your PHA. This online portion must be completed prior to scheduling. The provider portion will not populate until the service members’ online portion is complete.

Immunizations

Screenings and administration of immunizations are available on a walk-in basis.

Vision Screening
Hours: Monday–Friday, 7 a.m. to 3:30 p.m.
Vision readiness screenings are conducted by the Medical Readiness Clinic. If you wear contacts, remove them 24 hours prior to the screening and bring your glasses. Updating your vision does not require a visit to Optometry.
If you need a new set of glasses, mask inserts, or a new prescription, contact Optometry at 703-692-0963 to make an appointment.
Audiology
703-692-8963, Hours: Monday, Tuesday, Thursday, and Friday, 7:30 to 11 a.m. and 1 to 3:30 p.m.
Audiology is available by walk-in basis only. Final audiograms must be completed within 6 months of retirement/separation date. This department is also available for issues pertaining to hearing aid fittings, repairs, replacements and referrals. If you are on flight status, you can also call Flight Medicine at 703-697-3255.
Laboratory
703-692-8986/8990, Hours: Monday–Friday, 7 a.m. to Noon and 12:30 to 3 p.m. with limited services from 3 to 4 p.m. for STAT blood draw requests only
HIV is due every 2 years and can be drawn at a walk-in basis at our laboratory.
- PHA: During your appointment, the provider will determine what labs should be ordered.
- Deployment: Depending on the location you are going to; your HIV might need to be current within 6 months of your ship out date. Refer to your leadership for further guidance.
- Final Physical: If you are retiring or separating, there is a specific set of orders that are required. These labs are ordered by our staff when we schedule your final physical appointment and must be completed at least a week before your appointment, or your appointment will be rescheduled.
Dental

To schedule an appointment, or get a digital signature on paperwork, send all documents and requests to the Dental mailbox. The subject line of your email must clearly state the nature of your request, i.e. "Request Appointment" or the form number that must be signed, i.e. "2808","4036", etc.
Radiology
Hours: Monday–Friday, 7 a.m. to Noon and 1 to 4 p.m.
Radiology is available on a walk-in basis once an X-ray is ordered and signed off by a provider. After submitting your packet for retirement, a chest X-ray will be ordered by Medical Readiness staff at the time of scheduling your final physical.
Well Woman Exam
Schedule as needed with your PCM at 703-697-1111. A normal Pap smear with a negative HPV must be completed every five years.
Mammogram
If over the age of 40, schedule an appointment with your PCM to request a referral for a mammogram at 703-697-1111. Referrals expire after 28 days.
Colonoscopy
