Periodic Health Assessment

Directions for completing your Periodic Health Assessment at Alexander T. Augusta Military Medical Center (ATAMMC), formerly Fort Belvoir Community Hospital.

Prior to appointment you must complete the online PHA questionnaire appropriate for your branch of service

Air Force

• Complete the Air Force's PHA. You will receive a phone call within 72 hours to schedule an appointment

Army

• Complete the Army's online PHA. Call Central Scheduling for appointment, 301-319-6591

Navy & Marines

• Complete the Navy's online PHA. Call JMRC Front Desk for appointment, 571-231-REDI (7334)

Pertinent information for success

• As of January 8, 2021, all PHA appointments are virtual unless otherwise coordinated

NOTE: The results from the electronic health/risk assessment should be available in the AHLTA medical record within one week of completion. MEDROS and MRRS will update the Individual Medical Readiness (IMR) report within 72 hours. If the results have not correctly transferred one week after completion, please call 571-231-REDI (7334) to investigate the delay. Do not contact your Primary Care Manager.
