DiLorenzo Pentagon Health Clinic Primary Care

DiLorenzo Pentagon Health Clinic Medical Home (formerly known as the Primary Care Clinic) provides acute and routine care services to approximately 8,000 enrolled active duty and VIP personnel assigned to the Pentagon. We are a tri-service clinic in both patient enrollment and staff. We average about 2,100 outpatient visits monthly. The Medical Home is comprised of two teams, BURGUNDY and GOLD, each consisting of physicians and physician assistants (PAs), nurses, licensed practical nurses, medical assistants and clerks. The teams are directly supported by wellness, dietary, behavioral health, case management, pharmacy, administrative and other services. Active duty, dependents and retirees who are TRICARE prime eligible and work at the Pentagon are eligible for enrollment at DPHC.

The DPHC Medical Home is recognized by the National Committee for Quality Assurance (NCQA) as a Level III Patient-Centered Medical Home (PCMH). It is a member of the network of PCMHs in our sister facilities across the National Capital Region Network (NCRN). In the PCMH, patients are the center of a care plan, and high quality care is driven by a team of health care professionals (“patient-centered, provider-led, team-driven”). Emphasis is placed on access to care, visit continuity with one’s provider, and responsive services within an integrated referral network and patient satisfaction. For more information on PCMH, please visit https://pcmh.ahrq.gov/page/defining-pcmh.

Making Appointments

The Integrated Referral Management and Appointment Call Center (IRMAC) can assist you with scheduling your appointment when you contact them at 1-855-CAPMED1 (227-6331) from 6:30 a.m. to 4 p.m. OR you can also use TRICARE Online (TOL) Patient Portal at tricareonline.com, 24 hours a day, 7 days a week.

We offer several different types of appointments for various needs

  • ACUTE: scheduled within 24 hours for acute conditions that may worsen without treatment in the next 3 days.
  • ​ESTABLISHED: for evaluation and follow-up of new or chronic health problems, referrals, new medication requests and well-women exams.

Secure Messaging

TRICARE Online Secure Messaging connects patients with healthcare providers and teams and allows for the secure exchange of health information and communications. We strongly encourage all enrollees to register in TOL Secure Messaging so that you can communicate with your healthcare team. TOL Secure Messaging allows you to send a message anytime to your team. You can inquire about lab results, medications, symptoms, appointment availability, etc.

Telephone Consult (T-Con) Services and Secure Messaging

Not all health care requires an appointment with a provider. Certain requests can be sent to a provider in the form of a telephone consult, an electronic message transcribed in our medical computer system. T-Cons are initiated by calling the appointment line 703-692-8810. Your provider or nurse will contact you within three (3) business days. Please ensure your contact information is updated.

Requests that are appropriate for Secure Messaging or Telephone Consults include:

  • Obtaining lab and radiology results
  • ​Renewing expired prescriptions (some exclusions/conditions may apply)
    • Name, dose and frequency of medication MUST be known
    • Narcotics and controlled drugs are refilled only by appointment
  • Renewing an expired or updating authorization for an existing specialty consult/referral

Internal Behavioral Health Consultant

The DPHC Internal Behavioral Health Consultation service (IBHC) is a program for enrolled patients within the DPHC Medical Home. Our purpose is to assist with habits, behaviors, stress, worry or emotional concerns. We can help with many problems including smoking cessation, sleep, chronic pain, weight loss, alcohol use, depression, anxiety, anger issues, diabetes, hypertension, and much more! We can offer same day or the next day appointments and they are usually no longer than 30 min. The IBHC staff are the experts in behavioral treatment and medication adherence. We have team that consists of a licensed clinical psychologist and a registered nurse.

Nurse of the Day (NOD)

A triage nurse is available during normal duty hours to evaluate acute or urgent conditions on a walk-in basis (this has replaced traditional Sick Call). The NOD will assess your medical complaint and determine the appropriate course of care, whether it is scheduling a routine appointment, self-care recommendations, or a same day visit with a medical provider.

What to Bring to Your Appointment 

  • Medical record (if record is not maintained at DPHC)
  • List of current medications including name and dosage and refill needs
  • Other medical information (i.e. notes from emergency room)
  • List of written questions for your medical provider
